Epiphyseal Stapling in Lower Limb Length Discrepancy Associated with Neurofibromatosis -A Case Report-

Abstract
Limb length discrepancy of the lower extremity, unlike upper extremity, can cause serious disability either cosmetically or functionally Particularly in children having modified growth with underlying disease, it remains difficult to predict the discrepancy precisely after maturity and to select the best treatment for it. In tan. 1994, the authors experienced a case of leg length discrepancy of a 7-year-old boy associated with neurofibromatosis. His left lower extremity was much longer than his right extremity. The difference was 7.4cm. Epiphyseal stapling was done on the distal femur and proximal tibia on the left side. Radiologic examination was taken semiannually for 5 years to check the change of the length. On the patient¡¯s last visit in Jan. 1999, the discrepancy had decreased from 7.4§¯ to 2.3§¯ and the valgus angulation of the left tibia had decreased from 10 to 0 degrees. The discrepancy after maturity is predicted to be 1.7cm when calculated by arithmetic method. Epiphyseal stapling may be one useful method of treatment for marked leg length discrepancy in young children who have underlying disease such as neurofibromatosis.
